In the first facility, we observed a reliance on manual paper charts and fragmented data. Patients were frequently awakened in the middle of the night for routine vitals, and families were left with almost zero visibility into the treatment process.

While our team member was still in care, we deployed a prototype smart patient monitoring system designed to bridge the communication gap between medical devices, staff, and families.

• Visual Transparency: Real-time vital signs were pushed directly to the room’s LED TV, allowing the patient and family to see health data without waiting for a nurse.

• Wearable Integration: We seamlessly connected the system to a digital watch, tracking heart rate, oxygen levels (SpO2), and blood pressure 24/7.

• Automated Fluid Tracking: The system monitored catheter output automatically, removing the need for manual measurement and reducing human error.

• Cross-Facility Continuity: When the patient was transferred to a second hospital, the system moved with them—continuing to send data and critical warnings directly to the nursing station.

The Impact: Health Data That Saves Time (and Money)

The platform wasn't just about tracking numbers; it was about proactive intervention.

• Safety Alerts: The system detected a 1.5-hour medication delay. It immediately alerted the nursing station and notified the family via Email and Microsoft Teams, ensuring the error was corrected before it impacted the patient's recovery.

• Financial Transparency: We integrated a medication price comparison tool. By showing the price difference between hospital pharmacy rates and local retail stores, we helped the family achieve significant cost savings during a stressful time.
